HonorVet Technologies is seeking a travel nurse RN Case Management for a travel nursing job in Burbank, California.

Duration: 13 weeks
Shift: Day, 5x8-hour (08:00–16:30)
Weekend Coverage: Every other weekend (typically one full weekend per 4 weeks)
Orientation: 40 hours

Experience:

• Minimum 2 years of acute medical case management experience, focusing on care coordination and discharge planning.
• Travel experience required (first-time travelers considered).
• Experience with HMOs, IPAs, and computerized documentation required.

Education & Licensure:

• Current and active RN license (California).
• BLS certification (facility standard).

The Registered Nurse – Case Manager is responsible for coordinating and managing patient care throughout the continuum—from admission to discharge—at Providence Saint Joseph Medical Center, a major acute care facility in Burbank, CA. This role emphasizes discharge planning, care coordination, utilization review, and compliance with CMS and regulatory standards.

The ideal candidate has strong experience managing care for acute medical patients, familiarity with HMOs/IPAs, and proficiency with the Epic EMR system.

• Conduct comprehensive assessments to determine patients’ medical, functional, and psychosocial needs.
• Develop, implement, and monitor individualized plans of care.
• Coordinate with interdisciplinary teams to ensure effective discharge planning and continuity of care.
• Determine medical necessity based on evidence-based guidelines and regulatory standards.
• Manage utilization review and ensure compliance with CMS, DRG, and HIPAA requirements.
• Facilitate referrals for DME, home health, and community resources.
• Maintain accurate electronic documentation using Epic.

Preferred Skills:

• Knowledge of CPT coding, CMS regulations, DRG assignment, and discharge planning workflows.
• Familiarity with adult and geriatric patient populations.
