This vital field role involves providing direct patient care and managing cases within the community to optimize healthcare outcomes. This position offers a competitive salary, mileage reimbursement, and a $3,000 signing bonus after 90 days. The final hiring decision will be made by our client agency.

Responsibilities - Provide direct patient care and develop personalized care plans.

- Collaborate with healthcare professionals to ensure seamless care and effective case management.
Requirements - License: Active RN license in Nevada with no restrictions.
- Experience: At least 1 year of experience supporting in-home patients.
- Skills:
- Proficient in case management software
- Strong organizational and communication skills
